OMMA Patient Application Documentation Checklist

Use the following checklist to ensure we have all necessary documentation to submit your application on your behalf. 

How to Send Us Your OMMA Application Documentation

To get your OMMA documentation sent over to B2Bkush for application processing, please do one of the following: 

 

  1. If you are not tech savvy at all (or prefer to do this in the easiest possible way), gather all of the following documentation included in the following checklist, locate your nearest print shop, go there, and have them scan all everything from the following checklist, and email it to patientapplications@b2bkush.com.

    This is likely going to be your easiest and fastest option unless you have a full-color scanner that you know how to use.

  2. If you have access to a scanner and you know how to use it, do so to email all of the following documentation, in full color, to patientapplications@b2bkush.com

  3. If you are slightly tech savvy and you have an Android phone, download PhotoScan by Google Photos from the Google Play Store, then use it to scan all of the following documentation, and email it to patientapplications@b2bkush.com
  4. If you are slightly tech savvy and you have an Apple phone, download PhotoScan by Google Photos from the iTunes Store, then use it to scan all of the following documentation, and email it to patientapplications@b2bkush.com

 

Oklahoma Medical Marijuana Documentation Checklist

 

1. Proof of Oklahoma residency (digital copy of one of the following):   

  • *Oklahoma driver’s license (front and back)
  • *Oklahoma identification card (front and back)

    If you provide your Oklahoma driver’s license or Oklahoma identification card, it can be used for your proof of residency AND your proof of identity.  
  • Oklahoma voter identification card
  • A utility bill for the calendar month preceding the date of application, excluding cellular telephone and internet bills
  • A residential property deed to property in the State of Oklahoma
  • A current rental agreement for residential property located in the State of Oklahoma

*Recommended, if you have,

2. Proof of identity (digital copy of one of the following):  

  • *Oklahoma driver’s license (front and back) 

  • *Oklahoma identification card (front and back)

    If you provide your Oklahoma driver’s license or Oklahoma identification card, it can be used for your proof of residency AND your proof of identity.  

  • U.S. passport or other photo identification issued by the U.S government
  • United States Bureau of Indian Affairs identification card or a Oklahoma tribal photo identification cards: Muscogee (Creek) Nation of Oklahoma, Kiowa, Comanche, Apache of Oklahoma, Absentee Shawnee, Cheyenne-Arapaho, Choctaw, Delaware, Caddo Nation of Oklahoma issued on or after January 8, 2008.

*Recommended, if you have. 

3. A clear, color, full-face digital photograph

  • Have somebody take a photo of you with a solid background behind you (similar to how your ddriver’s license or passport photo would be taken). 

4. Adult Patient Physician Recommendation Form dated within 30 days of application submission

**Please note: The “Certifying Board” field on this form must be filled out in its entirety or the application will be rejected. Please ensure the physician fills this out using the full, complete name of his or her certifying board (e.g., “American Board of Family Medicine” instead of “Family Medicine”).**

5. If applicable, your Medicaid (SoonerCare) or Medicare insurance card or enrollment documentation

6. If applicable, your Adult Patient Caregiver Designation form
